​Claim denials are very common in the world of medical billing. Duplicate claim denials happen often. Within an electronic health record (EHR), if there are more than one patient chart per client, duplicate claim denials can occur. In an effort to identify claims that need to be reversed by the billing staff, claims that are waiting on a reversal, or when the patient chart is ready for merge into the original patient chart, a database design is being created.  This will aid the medical billing team in prepping claims before they can merge the clients together and rebill the claims properly from the same chart.
